When you need this by. A $50 rush fee may be added and billed separately if needed in less than 10 business days, depending on how busy we are at the time of your order. 
What unit or organization you are with 
What 2 matting colors would you like? See examples in photos; you will get a main color and a boarder accent color. 
A general idea of how you would like your plaque to look. Do you have coins, pins, ranks, photos or other personal items that you'd like included? 
Nameplate Verbage. There is no limit to how many words you can have but we suggest keeping it to 4-5 sentences.
 
I will respond to your email with a digital proof within 48 hours. Please look this over for any errors. I MUST RECEIVE A CONFIRMATION EMAIL REPLY before I put your item into production so it is imparative that you check your email for coorospondance. 
 
If you have items you would like to include in your plaque, I offer FREE pick up of these items on Tuesdays and Thursdays at an address of your choice on JBLM or within 10 miles.
